Prescription Refill and Lab Requests


Please log into the patient portal to request a prescription refill or communicate a message to one of the staff members. You may contact the office via phone to request refills, however the patient portal will provide a quicker response time.  Do not send an e-mail request.

Please be sure to bring your insurance card(s) and a valid picture ID that has your current mailing address with you to your first visit. If you forget to bring these items you will be asked to reschedule.

ABCDE's


If you have one or more of the ABCDE make an appointment with your dermatologist immediately

A – Asymmetrical: irregular in shape
B – Border: the border of the spot is irregular and not smooth
C – Color: it is not one even color, there is more than one color in the spot (blue, black, brown, tan, etc)
D – Diameter: if it is larger than an eraser head, 6mm
E – Evolution: if the spot has changed over time, and knowing what is normal for you and your moles or spots
